Microsoft Forms is free to use with a personal Microsoft account. If you need the benefits of an eligible paid Microsoft 365 plan, you pay for that subscription—not for a separate Forms plan. This guide explains how to choose a plan, enter a Coinepay virtual card at Microsoft checkout, and troubleshoot a declined payment.

Check whether you need to pay

Start by signing in to Microsoft Forms with your personal Microsoft account or eligible work or school credentials. Select New Form to create a form. There is no Forms-specific checkout at this step.

A free personal account can collect up to 200 responses per form; a paid personal account can collect up to 1,000. Microsoft publishes different limits for eligible organizational accounts. Check the current limits and your account’s eligibility before buying a subscription solely to increase response capacity.

For personal use, Microsoft lists Forms with Microsoft 365 Personal, Microsoft 365 Family, and Microsoft 365 Premium. For organizations, eligible options include Microsoft 365 Business Basic, Business Standard, and Business Premium, as well as specified education, enterprise, frontline, and OneDrive for Business plans. There is no separate paid subscription named “Microsoft Forms.”

At the time of writing (October 2026), Microsoft’s US personal-plan listing showed Personal at $9.99 per month or $99.99 per year, Family at $12.99 per month or $129.99 per year, and Premium at $19.99 per month or $199.99 per year. These are Microsoft 365 prices, not separate Forms charges. Prices, taxes, and available terms can differ by region; check Microsoft’s official pricing page and the final checkout total before ordering.

What you need before checkout

  • The right Microsoft account: Use the personal account that should own the subscription, or an authorized organizational account for a business purchase.
  • A plan available in your region: Review the Microsoft 365 options shown for your location. Use your real account, identity, and billing details; do not try to bypass regional or payment requirements.
  • A Coinepay account and virtual card: Register with Coinepay and complete identity verification before using card services. Your virtual card’s number, expiry date, and CVV are available when it is issued. Use the Coinepay app or dashboard to check its details and available funds.
  • Enough funds for the selected checkout total: Check whether you chose monthly or yearly billing, and account for the amount displayed by Microsoft, including any applicable taxes. Review upcoming renewal charges too.

Important for business purchases: Microsoft says business payment methods must be issued in the tenant’s country or region. It also rejects prepaid and stored-value cards, including gift cards. Contact Coinepay support to confirm your card meets Microsoft’s requirements before you buy. Do not assume that having a virtual card makes it eligible for a particular Microsoft business tenant.

How to pay for a personal Microsoft 365 plan

  1. On Microsoft’s official Microsoft 365 comparison page for your region, compare Personal, Family, and Premium. If free Forms meets your needs, you can skip the purchase.
  2. Select the plan you want and the monthly or yearly option offered at checkout. Check which account will receive the subscription before continuing.
  3. Sign in to your personal Microsoft account, or create one using your real details if you do not already have one.
  4. When Microsoft asks for a payment method, choose the card option available at your local checkout. Enter your Coinepay virtual card number, expiry date, and CVV exactly as shown in Coinepay. Enter your real billing details accurately.
  5. Review the plan, billing term, total due, and renewal terms. Complete any verification requested during checkout, then place the order.
  6. Check Services & subscriptions in your Microsoft account for the purchase. Sign in to Forms with that account and select New Form when you are ready to use it.

Microsoft lists credit cards, debit cards, and PayPal for personal Store checkout, but payment options can vary by region. The options presented in your checkout determine what you can try; entering a card does not guarantee Microsoft will accept it. Microsoft 365 subscriptions renew automatically unless cancelled.

How business checkout differs

An authorized buyer can select an eligible plan in the Microsoft 365 admin center, set the licence count, and choose the term and billing frequency where offered. The buyer then selects or creates a billing profile or payment method, reviews the order, and selects Place order. The path can differ between Microsoft Customer Agreement (MCA) and Microsoft Online Subscription Agreement (MOSA) billing accounts.

Before entering a Coinepay card, verify the tenant’s country or region and Microsoft’s business card restrictions with Coinepay support. If the card does not meet them, use another Microsoft-supported payment method available to your organization rather than repeatedly retrying it. Business accounts may also have invoice or selected local payment options, depending on the account and market.

If Microsoft declines the card

A decline may appear simply as “declined”, or Microsoft may email you to say it was unable to process the payment. Forms does not have a separate checkout error to troubleshoot: check the Microsoft 365 purchase and the payment method attached to it.

  • Insufficient balance: Compare your available funds with the full amount Microsoft displays, including applicable taxes. Check that you did not select yearly payment when you expected a monthly charge. If this is a renewal, review the amount due and any outstanding balance in your Microsoft account.
  • Card or billing-detail mismatch: Re-enter the number, expiry date, and CVV from Coinepay, and check for typing errors. Use your real billing address and make sure the details you submit are consistent with your payment information. An expired or inactive card can also cause a decline.
  • 3-D Secure or another verification request: If Microsoft presents an authentication step, complete it as instructed. If the step fails, times out, or never reaches you, do not assume the order went through. Check your Microsoft order or subscription status, then contact Coinepay support about the card verification attempt if needed.
  • Merchant region checks: Check the country or region of your Microsoft account, checkout, and billing details. For business accounts, remember Microsoft’s rule that the payment method must be issued in the tenant’s country or region, as well as its prohibition on prepaid and stored-value cards. Ask Coinepay support to confirm card eligibility rather than changing details to an address that is not yours.
  • Other payment blocks: Microsoft identifies bank blocking and outstanding balances among possible causes. Look for a payment notice in your Microsoft account. If the information is correct but the decline persists, contact Microsoft about the checkout and Coinepay support about the attempted card payment.

A failed checkout is not proof that you have a paid Forms entitlement. Confirm the subscription appears on the correct Microsoft account before relying on paid-account limits.

Manage the payment method and renewal

For a personal purchase, use your Microsoft account’s Payment options page to review payment methods and Services & subscriptions to review the subscription. To stop a personal subscription, go to Services & subscriptions → Manage → Cancel and read the terms shown before confirming. Refund eligibility depends on Microsoft’s policy, the purchase, and your region; cancellation does not automatically mean a refund.

Authorized business users can manage payment methods in the Microsoft 365 admin center under Billing → Payment methods or Bills & payments → Payment methods, depending on the view. Business cancellation and charges depend on the billing agreement and subscription term. Check those terms in the admin center before changing renewal settings.
